DHS BillingThe 4C of the U.P. offers billing technical assistance to licensed and registered providers and DHS funded day care aides/relative providers. This assistance includes help with the billing process to include Internet billing (I-billing) or Telephone Reporting, information on obtaining Provider/Client ID numbers and Personal Identification Numbers (PIN)as assigned by DHS (4C does not give ID# or PIN numbers), support in applying for internet billing, etc. Please contact us with any questions. If you need a PIN#, please call 1-800-444-5364, option 2 (M-F, except during holidays, 8:00 am-11:50 am and 1:00 pm - 4:50 pm Eastern Time) An alternative method is to fax a request to 517-335-6054 or 517-335-6236. Faxed requests must be signed and include your name, address, daytime telephone number and ID number. You also must include a copy of your drivers license or utility bill showing your current address. The address must match the address that Lansing has on file. You must report to DHS any address change within 10 days.
