DHS/CDC Funded Parent Reporting
Effective May 10, 2009 Parents receiving DHS Child Development and Care subsidy (financial assistance for child care) will need to report their actual child care hours every two weeks to DHS either by internet (I-billing) or telephone reporting. A parent will need to keep a daily record of actual activity hours on the DHS-641 CDC Parent Need Hours Worksheet. All hours reported to DHS MUST match what your child care provider is billing or this may delay payment to your child caregiver. 4C of the U.P. is offering technical assistance to parents to help with the reporting process and train on the internet I-billing system.
